If you have a question about a medical problem and are not sure if you need to see the doctor, please call 533-4146. The person who answers will help you decide if you should see the doctor, wait until the next day, or go to the hospital emergency room.

Refill a Prescription

To refill a prescription, call 574-534-3300 and enter 202 when the voice message says that you can enter an extension.

Be sure that your voice message includes:

  • Your first and last name
  • Your telephone number
  • The name of the medicine you need
  • Please spell the name of the medicine if you don't know how to pronounce it
  • The place where you want to pick up your medicine

Choosing One Person to Help You

When you become our patient, we will ask you to decide which doctor or nurse practitioner you want to help you. After you choose this helper, you should try to schedule all of your appointments with this person. You may schedule an appointment with a different helper if you are sick and your helper has a day off or is on vacation.

Fees

Fees at the Center are the same as other doctor’s offices, except that we offer a “sliding fee scale” so that your fee depends on your income. Discounts may be as much as 90% of the full fee. Each year, we will ask you to give us an update on your income so that your discount stays correct.
