Our patients live in east Goshen or north Goshen.
If you are our patient, feel free to call when you have questions or when you need advice. If we help you by phone, it saves you time and money.
Appointments
To see the doctor or nurse practitioner, call or stop in to make an appointment.
Please be on time for your appointment. If you cannot come to your appointment, please call the day before so that we can give your spot to someone else.
If you are more than 15 minutes late, you may lose your appointment.

Monday....8:00 a.m. to 12:00 p.m and 1:00 pm to 5:00 p.m.
Tuesday....8:00 a.m. to 12:00 p.m and 1:00 pm to 5:00 p.m.
Wednesday....CLOSED
Thursday....8:00 a.m. to 12:00 p.m and 2:00 pm to 5:00 p.m.
Friday....8:00 a.m. to 12:00 p.m and 1:00 pm to 5:00 p.m.
Saturday....CLOSED
Sunday....CLOSED
What to Do if We Are Closed
If you have a question about a medical problem and are not sure if you need to see the doctor, please call 533-4146. The person who answers will help you decide if you should see the doctor, wait until the next day, or go to the hospital emergency room.
Choosing One Person to Help You
When you become our patient, we will ask you to decide which doctor or nurse practitioner you want to help you. After you choose this helper, you should try to schedule all of your appointments with this person. You may schedule an appointment with a different helper if you are sick and your helper has a day off or is on vacation.
Fees
Fees at the Center are the same as other doctor’s offices, except that we offer a “sliding fee scale” so that your fee depends on your income. Discounts may be as much as 90% of the full fee. Each year, we will ask you to give us an update on your income so that your discount stays correct. (See Paying for Your Health Care for details about payment.)
